By accessing or using the Nexus Health Customer Portal ("Portal"), located at portal.nexushealth.me, you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, you may not access or use the Portal.

These Terms constitute a legally binding agreement between you and Nexus Health, Inc. ("Nexus Health", "we", "our", or "us").

Nexus Health provides precision wellness services through AI-driven clinical intelligence systems, including:

• BaselineONE™ – Clinical workflow management, reporting, and quality assurance tools for healthcare providers.
• BioMatrix™ – AI-powered health analysis, interpretation, and personalized supplement protocol generation.
• CSE™ (Couples Simulation Engine) – Relationship and family health optimization modeling.

The Portal provides access to upload health data, view reports, communicate with clinicians, and manage your wellness journey.

3. Eligibility

You must be at least 18 years of age to use the Portal. By using the Portal, you represent and warrant that you meet this age requirement and have the legal capacity to enter into these Terms.

Access for minors requires verified guardian consent and must be facilitated through an authorized clinician.

The information provided through BioMatrix™, BaselineONE™, or any other Nexus Health system should be used in conjunction with, not as a replacement for, professional medical judgment. Never disregard professional medical advice or delay seeking it because of information obtained through the Portal.

• Upload false, misleading, or fraudulent health information.
• Violate any applicable local, state, national, or international law.
• Infringe upon or violate our intellectual property rights or the rights of others.
• Transmit any harmful code, viruses, or malicious software.

• Use the Portal for any unlawful or prohibited purpose.
